I've done some more work on this since I've got back.
I've added a box on the toolbar to set the scale (paper scale), and also
a button to turn on intelligent selection of pages.

You can turn particular pages to print on and off with a right mouse
click (no drag). All seems to work fairly well.

I'll send the source code to Olly - do you want a diff against 1.1.3?
