Convert 3d --> dxf

At present, cad3d doesn't handle surface data specially.  Probably the
simplest solution for now is to put surface data in a separate layer
so it can be removed if you don't want it (like we do for names and
station markers).  How does that sound?

SpeleoGen doesn't understand modern 3d files.  I asked Mike about
rebuilding it to fix this, but he wasn't able to (I don't recall why).
Since it's now essentially useless, I've just removed it from the
"Related Tools" page (and fixed a few other things).
