Peli case, which is waterproof enough, which I added straps to, and
keep it attached to my belt almost every caving trip (the phone is my
camera and surveying tool). Inside that, I have the phone in a Unicorn
Beetle Pro SUPCASE (the type with built in screen protector). I don't
bother with a flexible waterproof pouch.

I poke holes in the case and thread a short loop of string through it
in a position which won't damage the phone, then attach that with a
clip to a string around my neck, so I can't drop it too far.

I use a simple capacitance stylus with a fine-ish point, since my
phone does not have a dedicated stylus. Samsung Galaxy Note has a
built in stylus with a really fine point, which would be better.
